For your convenience, we have two useful forms available for download.

The first is a decontamination form needed if mailing pipettes used with radioactive or bio-hazardous materials. If L&J Calibrations recieves any contaminated pipettes we will send them back at the owners expense. Download this form if it is needed to confirm the safety of your pipettes.

The second form available is our service request form. It is a downloadable form that you can mail or fax us to request one time or continual service for your pipettes. Feel free also to call or email us with a service request.
